E-Statement Authorization for Account Analysis Statements

I authorize Orange Bank & Trust to set up account analysis statement(s) to receive my monthly account statement(s) and federally mandated disclosures electronically.

By indicating below that you accept the terms of this agreement, you understand, agree and consent to receive your statements electronically, and will not receive paper statements in the mail for your Orange Bank & Trust analysis account (s). You are responsible for providing us with updated information that we need to contact you electronically, including any change in your e-mail address.

Statement Delivery Agreement

ONLINE STATEMENT DELIVERY SERVICE AGREEMENT

We offer Online Statement Delivery Service (the Service) for certain Eligible Accounts, allowing you to replace your mailed (paper) statement with an electronic version (PDF) that you can view through Online Banking. You may elect to save the statement to your computer, or print a copy. We may, at our option, offer you additional options and preferences for the delivery of various types of communications related to your Eligible Accounts or Online Banking services. From time to time, we may also add to, modify, or delete any feature of online statements in our sole discretion.

  1. ENROLLMENT IN ONLINE STATEMENTS
    You must be enrolled in Online Banking and accept the terms and conditions in the Treasury Cash Management Agreement. By enrolling in the Service you agree to be bound by the terms and conditions of this Online Statement Delivery Service Agreement and comply with all applicable federal state, and local laws and regulations. Acceptance and use of the Service signifies your receipt and understanding of the terms of this Agreement, and that the Agreement applies to all accounts you choose to include in the Service. For deposit accounts with multiple owners, you also represent and warrant that you are authorized to accept the terms of this Agreement on behalf of all Owners of the accounts enrolled in the Service, Please note that only the account owner must enroll the account in Online Statement Delivery. The Statement will be available to all users that have access to the account.
  2. DESCRIPTION OF SERVICE AND ELIGIBLE ACCOUNTS
    Once you select online statement delivery, you may view an electronic version of your account analysis statement for up to 36 months. This Service is provided to you free of charge and will be available at the next statement cycle.  Once you enroll in the Service, Orange Bank & Trust will not provide you with a paper statement. Electronic copies of your checks are available online and may be displayed or printed. You may also receive legal notices, disclosures and other important information through Online Banking. We may communicate with you by sending you an e- mail to the e-mail address of record, sending you a secure message through Online Banking or posting the information on the Web.
  3. NOTIFICATION OF CHANGE STATEMENT DELIVERY METHOD
    You may change the way you receive your analysis statement at any time by contacting us. You will not be charged a fee if you choose to change your delivery method. You may also withdraw consent and request that future statements be provided as a paper document delivered via the U. S. Postal Service at your address as reflected on our records, contact Orange Bank & Trust at 845-341-5120 or contact is through secured message via On line Banking.

We always reserve the right to communicate with you in writing using the U.S. Postal Service, no matter what other options you select.

  1. TERMINATION OF SERVICE
    You may elect to terminate the Service at any time by electing to receive a paper statement.

We may terminate the Service if you do not access your Eligible Accounts through Online Banking for any consecutive six month period.

Electronic Statements Disclosure

To sign up for E-Statements/Notices you must be an owner of the account(s) you have chosen for E-Statement service. You must also read and accept the Terms of Agreement below.

1. Scope of Communications to Be Provided in Electronic Form. By completing the application form, you agree that we may provide you with any communications, including but not limited to notices and disclosures that we are required by law to provide to you in writing, relating to that Account in electronic format. We may discontinue sending paper communications to you, unless and until you withdraw your consent as described below.

2. Method of Providing Communications to You in Electronic Form. All communications that we provide to you in electronic form will be available via e-mail at the e-mail address you provide us.

3. How to Withdraw Consent. You may withdraw your consent to receive communications in electronic form for this account(s) by contacting us via e-mail at operations@orangebanktrust.com,  via secured message in On Line Banking or contacting us at 845-341-5120. We may treat your provision of an invalid email address or the subsequent malfunction of a previously valid address as a withdrawal of your consent to receive electronic communications. We will not impose any fee to process the withdrawal of your consent to receive electronic communications. Any withdrawal of your consent to receive communications will be effective only after we have a reasonable period of time to process your withdrawal. If you do not receive an electronic communication from us at the end of the statement cycle (e-statement notice) and you did not intend to withdraw your consent, please contact us immediately to update your contact information according to one of the methods listed.

4. How to Update Your Records. It is your responsibility to provide us with a true, accurate and complete e-mail address, contact, and other information related to this Disclosure and your Account(s), and to maintain and update promptly any changes in this information. 6. Requesting paper Copies. We will not send you a paper copy of any statement or notice from us, unless you request it or we otherwise deem it appropriate to do so. You can obtain a paper copy of an Electric Statement or notice by printing it yourself or by requesting that we mail you a paper copy. To request a paper copy, contact us by telephone at 845-341-5120. We reserve the right, but assume no obligation, to provide a paper (instead of electronic) copy of any communication that you have authorized us to provide electronically.

7. Communications in Writing. All communications in either electronic or paper format from us to you will be considered "in writing." You should print or download for your records a copy of this Disclosure and any other communication that is important to you.

8. Federal Law. You acknowledge and agree that your consent to electronic communications is being provided in connection with a transaction affecting interstate commerce that is subject to the Federal Electronic Signatures in Global and National Commerce Act, and that you and we both intend that the Act apply to the fullest extent possible to validate our ability to conduct business with you by electronic means.

9. Termination/Changes. We reserve the right, in our sole discretion, to discontinue the provision of your electronic communications, or to terminate or change the terms and conditions on which we provide electronic communications.
