Disclosures

• You have the right to have your statement provided in a non-electronic form (paper).
• You have the right to withdraw your consent to have your statement provided electronically at any time with no fees.
• Your consent to receive your statements electronically via ISPFCU’s E-Statements does not apply to any other disclosures, forms, or services.
• You must provide a valid email address for delivery of your ISPFCU E-Statement.
• If you wish to withdraw your consent to receive your statement electronically please contact ISPFCU.  You can fax your request to 866-523-8756 or mail it to 730 Engineering Avenue, Springfield, IL  62703.
• You may obtain a paper copy of your statement upon request.
• You must have an email provider, internet access, and Adobe Acrobat to access and retain your ISPFCU E-Statements.
